Health Records
All students must meet health standards as dictated by the Commonwealth of Massachusetts. All full-time students (including full-time graduate students) must submit a completed student health history and immunization report form to BSU Health Services. Massachusetts immunization laws apply to full-time students of all ages. Proof of immunization must be provided by a physician or a prior school, and must include the dates (month and year) of:

One dose of tetanus/diphtheria/pertussis (TDap) within the last 10 years, OR one dose of tetanus/diphtheria (Td) within the last 5 years

Three doses of Hepatitis B (Hep B)

Two doses of measles, mumps, and rubella (MMR) after the patient's first birthday and after the year 1967

One dose of meningitis vaccine (Menactra) within 5 years; OR a signed waiver

Proof of Chicken Pox Disease (including year); OR one dose of Varicella vaccine

Annual Tuberculosis Test (may be done at BSU Health Services)

Annual Influenza Vaccination
Serologic proof of immunity will be acceptable. Immunization requirements apply as well to all international students attending or visiting classes as part of our academic exchange program.
